- 5-Year Renewed Certificate of Permission for Use.
- Permanently ceases the use of the equipment capable of producing ionizing radiation and disposes of it by transfer.
- Permanently ceases the use of the equipment capable of producing ionizing radiation and disposes of it by discards it.
- Application for a certificate of permission or registration for filing and reference for the use of radioactive material or equipment capable of producing ionizing radiation.
- Radiation safety survey report.
- Workplace radiation safety assessment.
- Radiation protection plan and radiation safety operation guidelines.
- The evaluation for the workplace radiation safety of the radioactive materials or the equipments capable of producing ionizing radiation.
